Payment and Insurance Information for InfiniteHeart Health, PLLC Patients
InfiniteHeart Health, PLLC, does not directly bill insurance companies or any other third-party payors for its services. However, all services provided by InfiniteHeart Health, PLLC, are billable. After each visit, you will receive a detailed "Superbill," which serves as a receipt that includes diagnostic (ICD) and treatment (CPT or HCPCS) codes. These codes are required by insurance companies to process claims and issue potential reimbursements. You may submit the Superbill to your insurance company to seek reimbursement for out-of-network services. The reimbursement rates depend upon your specific insurance policy and are not guaranteed. Please be aware that Medicare does not reimburse for out-of-network services, so Superbills cannot be submitted to Medicare or any other government programs for this purpose. InfiniteHeart Health, PLLC, does not guarantee payment by your insurance company for any services provided to you.

Coverage for Laboratory Tests, Diagnostics, and Medications
While InfiniteHeart Health, PLLC’s services are out-of-network, all laboratory tests, diagnostic tests, procedures, and prescribed medications will still be submitted to your insurance company. These services will be processed in accordance with your insurance company’s requirements and such services may be covered.
